Antique U.S. Internal Revenue booklet of “Order Forms for Opium, Etc.” issued under the Act of Congress approved December 17, 1914. Contains unused original and duplicate forms numbered X771651 to X771660, assigned to H. P. Buell of Colchester, Connecticut, with some entries typed “By Earl R. Holmes.” Forms are Class 3 narcotic order forms intended for the lawful procurement of opium and related preparations by registered professionals.

The booklet includes pink duplicate forms, black-bordered originals (each marked “Value, One Cent”), printed instructions, and the original U.S. Internal Revenue cover. A rare surviving example of early federal narcotics control paperwork from the period immediately following passage of the Harrison Narcotics Tax Act. Light wear to covers; interior forms clean and unused.
